The short version
The simplest way: dip the shower head in a bag with citric-acid or vinegar solution, let it act for an hour, rub off the nozzles, done. For chrome parts prefer citric acid, it is gentler.
Limescale clogs the fine nozzles of the shower head, the jet becomes uneven and weak. Descaling restores the full flow and saves water. Whether removable or fixed, both are surprisingly easy.

Make the solution
Dissolve citric acid in warm water or mix vinegar 1:1 with water, put it in a sturdy bag or a bowl.
Submerge the head
Lay a removable head in, wrap a fixed head with the bag and fix it with a rubber band so the nozzles are covered.
Let it act
Leave for about an hour, longer for heavy scaling. Do not boil or heat.
Rub the nozzles & rinse
Rub the loosened scale off the nozzles with a finger or an old toothbrush, then rinse thoroughly with clear water.
Mind the material
For chrome, plastic or coated nozzles prefer citric acid over harsh vinegar, and do not let it act too long. Some shower heads have anti-scale nubs you can simply rub off.
